ENROLLMENT PROCESS

At Tree Of Life Christian Academy we make enrollment easy for our families. With a simple registration fee your child’s space is held.

FORMS NEEDED AT TIME OF REGISTRATION:

1. Preschool health forms, Blue & Yellow (your pediatrician has these  forms & will fill them out for you). Immunization form must be updated at time of registration.

2. Enrollment & Tuition forms completed & signed.

3. Signed Parent Handbook acceptance page

4. Authorization pick up form

5. Identification / FL Drivers License
